IDD MHHI Summits are held annually with the goal of advancing the development of policies, programs, and practices that promote integrated person-centered care for people with Intellectual and Developmental Disabilities and their families. National experts with direct experience in the development of integrated health care for people with I/DD have shared their models, recommendations, and lessons learned. The Summits have also highlighted innovative projects underway in our state that are contributing to systems change and support the goals of Medicaid Transformation. The focus of previous years' Summits has been:

• 2015: Four State Approaches to Integrated Care for People with I/DD

• 2016: Advancing Integrated and Collaborative Care for People with I/DD
